This package consists of six programs that offer updates for physicians when treating a variety of patient safety issues. The topics included are Prevention of Medical Errors, Child Abuse Prevention, Antibiotic Stewardship, Elder Mistreatment, Medication Safety & Pharmacology Updates, and Management of the Poisoned Patient. These faculty are from all over the country and offer great practical information and updates on keeping patients safe. 

Accreditation Statements:
This program is co-sponsored by the Missouri Association of Osteopathic Physicians and Surgeons (MAOPS) and the Osteopathic Physicians and Surgeons of Oregon (OPSO). Both osteopathic associations are accredited by the American Osteopathic Association to provide osteopathic continuing medical education for physicians.

The program has been designated for a maximum of 6 AOA Category 1-A Credit(s) by the co-sponsors who have authorized the Missouri Association of Osteopathic Physicians and Surgeons to report CME and specialty credits to the AOA commensurate with the extent of the physician's participation in this activity.  

The Missouri Association of Osteopathic Physicians and Surgeons (MAOPS) is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.

Grievance Policy:

Dr. Yvonne Cheung is the Associate Chief Medical Officer and Associate Chief Quality Officer at Baystate Medical Center in Springfield, Massachusetts. She also serves as Chair of the Quality and Patient Safety Committee of the Massachusetts Board of Registration in Medicine (BORM). She is also a staff anesthesiologist at Springfield Anesthesia Services. Dr. Cheung’s prior leadership roles in Quality and Patient Safety include Chief Quality and Safety Officer at Mount Auburn Hospital in Cambridge, Massachusetts, and Associate Chief Medical Officer/Vice President of Quality and Patient Safety at Mass General Brigham | Newton-Wellesley in Newton, Massachusetts. 

Dr. Cheung received her Bachelor of Arts from Harvard University in Cambridge, Massachusetts. She received her medical degree from Columbia University College of Physicians and Surgeons in New York City, New York. She obtained an MBA in Medical Management from the University of Massachusetts, Amherst Isenberg School of Management in Amherst, Massachusetts. Dr. Cheung completed her internship in Internal Medicine at Mount Auburn Hospital in Cambridge, Massachusetts. She then completed her residency in Anesthesiology at Massachusetts General Hospital, her fellowship in Quality and Patient Safety in 2014, and another fellowship in Medical Education in 2016 at Harvard Medical School in Boston, Massachusetts. Dr. Cheung is a diplomate and is board-certified in Anesthesiology by the American Board of Anesthesiology.

Dr. Cheung is on the Board of Directors for the Massachusetts Coalition for the Prevention of Medical Errors. She is a member of several professional associations, including the American Society of Anesthesiologists, the Massachusetts Society of Anesthesiologists, and the Massachusetts Medical Society. The governor appointed her as a physician member of the Massachusetts Board of Registration in Medicine Board.

Christopher Holstege, MD's Profile

Dr. Christopher Holstege is an Emergency Medicine and Pediatrics professor at the University of Virginia Professor School of Medicine in Charlottesville, Virginia. He is also the Chief of the Division of Medical Toxicology and the Senior Associate Vice-President of Student Health & Wellness. Dr. Holstege is a practicing emergency medicine faculty member who provides clinical care for poisoned patients at UVA Health. He is leading efforts in the care, health surveillance, and protection of all students at the University of Virginia, especially pertaining to student substance use. Dr. Holstege received his Doctor of Medicine from Wayne State University School of Medicine in Detroit, Michigan. He completed his residency in Emergency Medicine at Butterworth Hospital in Grand Rapids, Michigan. He completed his fellowship in Medical Toxicology at the Indiana University School of Medicine in Indianapolis, Indiana. Dr. Holstege is board-certified in Emergency Medicine and Medical Toxicology by the American Board of Emergency Medicine. In addition, he is a diplomat of the National Board of Medical Examiners. 

Dr. Holstege has numerous ongoing research projects in clinical, basic science, and epidemiologic arenas. Dr. Holstege has over 300 publications in various medical journals, periodicals, and books. He received the Calvin University Distinguished Alumni Award in 2022 and The Raven Award in 2021, given annually by the Raven Society to recognize excellence in service and contribution to the University of Virginia. In addition, he received the American Red Cross Christopher E. Lee Humanitarian Spirit Hero ward in 2021, which is given annually to a community member who exemplifies a humanitarian spirit and servant leadership. 

Dr. Holstege is married and has six children: Erik, Elijah, Benjamin, Samuel, Noah, and Annalee.

Terra N. Frazier, DO's Profile

Dr. Terra Frazier is a Professor of Pediatrics in the Division of Childhood Adversity and Resilience at Children’s Mercy Hospital University of Missouri-Kansas City School of Medicine in Kansas City, Missouri. She also serves as Associate Fellowship Program Director. She works full-time in the field of Child Abuse Pediatrics in both inpatient and outpatient settings. Dr. Frazier completed medical school at Oklahoma State University College of Osteopathic Medicine in Tulsa, Oklahoma. She completed her residency in Pediatrics and her fellowship in Child Abuse Pediatrics at Children’s Mercy Hospital and Clinics in Kansas City, Missouri. Dr. Frazier is board-certified in General Pediatrics and Child Abuse Pediatrics by the American Board of Pediatrics.

Dr. Frazier serves on many different multi-disciplinary teams related to child protection in Kansas and Missouri, as well as the Missouri State Child Fatality Review panel. She enjoys educating others about the medical aspects of child abuse and has a strong interest in ways to improve communication and build partnerships among multi-disciplinary team members. She is a member of the Ray E. Helfer Society, an international honorary society of physicians seeking to provide medical leadership regarding prevention, diagnosis, treatment, and research concerning child abuse and neglect. 

Dr. Frazier received the Golden Apple Mercy Mentor award in 2016, 2021, and 2024 from the graduating fellows at Children’s Mercy Hospital. This award is given to faculty members who have significantly contributed to the growth and development of the fellows during their fellowship training. She also received the Leavenworth County Child Advocate of the Year award in 2022 from the Leavenworth Child Advocate Center.

Sara Keller, MD, MPH, MSPH's Profile

Dr. Sara Keller graduated from the Duke University School of Medicine in Durham, North Carolina, in 2007. She completed her Masters in Public Health in Epidemiology in 2006 at the University of North Carolina at Chapel Hill. Dr. Keller then completed her residency in Internal Medicine in the Oster Medical Residency Training Program at the Johns Hopkins Hospital in Baltimore, Maryland, in 2010. Dr. Keller then completed her fellowship in Infectious Diseases, Healthcare Improvement and Patient Safety in 2013 at the Centers for Healthcare Improvement and Patient Safety, Hospital of the University of Pennsylvania, in Philadelphia. She also completed a Master of Science in Health Policy at the University of Pennsylvania in Philadelphia in 2013. 

Dr. Keller is an Associate Professor in the Division of Infectious Diseases, Department of Medicine at the Johns Hopkins University School of Medicine in Baltimore, Maryland. She has joint appointments in the Department of Health Policy and Management at the Johns Hopkins Bloomberg School of Public Health and the Armstrong Institute for Patient Safety and Quality at the Johns Hopkins Bloomberg School of Public Health. Dr. Keller also directs the Johns Hopkins Internal Medicine Residency Program’s Patient Safety and Quality Improvement Pathway. Dr. Keller is a practicing infectious diseases physician. Dr. Keller is board-certified in Internal Medicine and Infectious Disease by the American Board of Internal Medicine. 

Dr. Keller’s research interests include healthcare-associated infection prevention in home settings, outpatient parenteral antimicrobial therapy, and improving antibiotic stewardship, particularly in ambulatory and telehealth settings. She has led national collaboratives focusing on improving appropriate antibiotic use in primary care, urgent care settings, and telemedicine settings. Her primary research focus is on improving the quality of care and patient safety among patients being discharged from the hospital on intravenous antimicrobials or with venous catheters in the home. 

Dr. Keller is a member of the Infectious Disease Society of America and the Society of Healthcare Epidemiology of America. In 2023, the Society for Healthcare Epidemiology of America named her a Super Advocate. Dr. Keller became a fellow of the Infectious Disease Society of America in 2022.

Sonia Sehgal, MD's Profile

Dr. Sonia Sehgal is a Clinical Professor in the Department of Internal Medicine and the Division of Geriatric Medicine and Gerontology at the University of California, Irvine. She leads as the Assistant Chief and the Director of Medical Education for the Division of Geriatric Medicine and Gerontology. As a medical educator, Dr. Sehgal is a UC Irvine School of Medicine leader. She is the Director of the Geriatric Medicine Fellowship and a Deans’ Scholar. Dr. Sehgal earned her Fellowship in Geriatric Medicine at UCI Medical Center. Dr. Sehgal cares for older adults to promote optimal living and empowered longevity at the UCI Health Senior Health Center in Orange County and UCI Health in Laguna Hills. Dr. Sehgal is board-certified in Internal Medicine and Geriatric Medicine by the American Board of Internal Medicine. She specializes in the care and treatment of older adults.

Dr. Sehgal’s clinical interests include healthy aging, lifestyle medicine, hypertension, osteoporosis, preventive medicine, and elder abuse. She maintains some of the highest patient satisfaction scores at UC I Health. Furthermore, she has been honored as a Physician of Excellence by the Orange County Medical Association yearly since 2012, recognizing her commitment to providing quality geriatric care to the Orange County community. Dr. Sehgal received the Excellence in Teaching Award annually from 2008-2016 and the MVP Outstanding Service Award from the Office of Educational Affairs of UCI School of Medicine. Dr. Sehgal also received the Gold Star Award from the Gold Humanism Honor Society.

In her research and leadership, she oversees educational initiatives in federally-funded research initiatives. Over the past seven years, she has served as the Director of Curriculum for the UC Irvine Geriatrics Workforce Enhancement Program (GWEP, HRSA). She educates the community physicians in the specialized care of older adults and oversees the geriatric curriculum for residents, medical students, and nursing students at UC Irvine. With passion and expertise, she is on the cutting edge of pedagogy nationwide, leading UCI’s GWEP Geriatric educational ECHO series on Age Friendly Health Systems utilizing the 4-M curriculum, What Matters Most, Mentation, Medication, and Mobility.

Cassandra White, PharmD, BCACP, BCGP's Profile

Dr. Cassandra White is a 2012 graduate of Albany College of Pharmacy and Health Sciences in Albany, New York. She is native of Hermon, Maine, and is proud to be a licensed pharmacist and Board of Pharmacy member in her home state. Throughout her 20-year pharmacy career, she has experienced working for RIte Aide, CVS, Walmart, and as an Associate Professor of Pharmacy Practice at Husson University for 11 years. She also practiced as a clinical pharmacist at Maine Dartmouth Geriatric Medicine and the Spinal Cord Injury Clinic at the VA Maine.  

Dr. White was the 2024 recipient of the Bowl of Hygeia Award for Maine. She spent 6 months of 2024 living her best life in "micro retirement" and completed a 5,609-mile road trip in June to achieve her lifelong dream of visiting all 50 states in the United States.
